Doing Business the Right Way
If you are a new patient, welcome. Here you will find the necessary resources so you are prepared with your first appointment. On making your initial appointment there will be a confirmation email sent with your appointment time and date that you have selected.
New Patient Information
Please take 5-10 minutes to complete the form online prior to your session.
Please arrive 5 minutes before your session time.
If you do not fill out the new patient form online, please arrive 15 minutes early to your session.
During the appointment you and your practitioner will go over the information together and begin the treatment.
The treatment will last about 45 minutes to 1 hour.
When finished, your practitioner will answer any questions. As well as go over the most affordable treatment care plan for you and scheduling your next visit.
